Innerbloom Medical offers one of the few physician-supervised couples ketamine therapy experiences in the Scottsdale area. Both partners receive intramuscular ketamine simultaneously in a shared, intentional setting — creating a rare window for connection, openness, and honest communication.

What Couples Ketamine Therapy Is

In a couples ketamine session at Innerbloom, both partners receive intramuscular (IM) ketamine simultaneously. IM administration produces a shorter, somewhat less intense experience than IV infusion — typically 45 to 90 minutes — which is well-suited to a shared relational context. The session is conducted in a private space with both partners present and supervised by Dr. Zabel throughout.

This is not a recreational experience. It is a clinical intervention, and it is taken seriously as such. Both partners undergo individual physician intakes prior to the session. Medical appropriateness is evaluated independently for each person. If one partner is not a candidate, the session does not proceed.

The Neuroscience of Relational Openness

Ketamine's effects on the default mode network — the neural circuitry associated with self-referential thought and rumination — are well documented. At therapeutic doses, ketamine temporarily reduces the rigid, self-protective patterns of thinking that often sustain conflict and disconnection in relationships. Emotional defensiveness decreases. Couples ketamine therapy is best suited to partners who are already engaged in couples therapy and feel that progress has stalled. It is also appropriate for couples where one or both partners have individual diagnoses of depression, anxiety, or PTSD that may be affecting the relationship dynamic.

Licensed couples therapists are welcome to refer their clients to Innerbloom for this service. Dr. Zabel handles the medical evaluation and clinical administration. The therapist remains the primary relational guide and continues integration work with the couple before and after the session. We are happy to communicate with referring therapists as appropriate, with patient consent.

Is this safe for both of us?

Safety is evaluated individually for each partner during the physician intake. Ketamine has a well-established safety profile when administered by a qualified physician. Dr. Zabel reviews each partner's medical and psychiatric history, current medications, and any contraindications before proceeding. The session is supervised by Dr. Zabel throughout.

Do we both need a medical evaluation?

Yes. Both partners require an individual physician intake prior to the couples session. These are conducted separately to ensure that each person's history is evaluated independently. If one partner is not a candidate — due to a contraindication or other clinical concern — the couples session does not proceed.

What if one of us is not a good candidate?

If one partner is not medically appropriate for ketamine therapy, we will discuss alternatives. Depending on the reason, there may be options such as individual IV ketamine therapy for the qualifying partner, Spravato for a partner who meets treatment-resistant depression criteria, or referral to other appropriate resources. We do not offer the couples session if either partner cannot safely participate.

Can we use our existing therapist for integration?

Yes — and we encourage it. The couples ketamine session is most valuable when embedded in an ongoing therapeutic relationship. If you are working with a couples therapist, we recommend scheduling a session with them in the days following the ketamine experience to process what emerged. With your permission, Dr. Zabel can communicate with your therapist to coordinate care.

How many sessions do couples typically do?

Most couples begin with a single session and assess from there. Some couples find one session produces meaningful shifts that they then develop through ongoing therapy. Others return for additional sessions over weeks or months. The frequency and total number of sessions is determined collaboratively based on your goals and response to the first session.
